RN333288L, of Philadelphia, Philadelphia County, based upon her being unable to practice with reasonable skill and safety to patients by reason of mental or physical illness or condition or physiological pr psychological dependence upon alcohol, hallucinogenic or narcotic drugs which tend to impair judgment or coordination.
