On March 6, 2014, the PA State Board of Nursing indefinitely suspended retroactive to October 9, 2013, the license of David Lewis, license no. PN095457L, of Philadelphia, Philadelphia, County, based upon findings he is unable to practice with reasonable skill and safety by reason of mental or physical illness or condition or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs that may impair judgment or coordination.
